ACP Traffic Lights

Prognosis < 6 months - 1 year

Introduce the notion of Advance Care Planning

Other things to consider:

  • GP Practice palliative care register
  • Nominate Key worker
  • DS1500 (< 6 months)
Prognosis of weeks

Review any existing Advance Care Plans, or

Encourage patient to discuss their wishes, especially consider:

  • Preferred Place of Care
  • Emergency Treatment plan for any anticipatable emergencies

Other things to consider:

  • Update out-of-hours service
  • Targeted Anticipatory prescribing or "Just in Case" box
Prognosis of days

Review any existing Advance Care Plans.

If not already discussed, consider especially:

  • Preferred Place of Care
  • Emergency Treatment plan for any anticipatable emergencies

Complete a DNA-CPR form

Other things to consider:

  • Last days of life Care Priorities
  • Update out-of-hours service 
  • Anticipatory prescribing or "Just in Case" box
