Consider unified assessment for more complex cases:
At less than 12 months/weeks (assessment of phase of illness: stable, unstable, deteriorating or dying) consider continuing healthcare application: Unified Assessment in Wales
Consider Specialist Palliative Care Referral as Appropriate:
