Primary care Gold standards framework and/or palliative care register
Making sure those in need have access to palliative care:
- A process must be in place which ensures that patients will receive timely and appropriate palliative care assessment and services.
Consider those patients experiencing unstable episodes or frequent exacerbations resulting in new unexpected problems or a rapid increase in the severity of existing problems.
A structured approach to achieving and monitoring this may be via a palliative care register and/or the Gold Standards Framework.
